Administration
This unit is responsible for mobilizing those funds accessed for programs and services, and also provides administrative and technical advice to the leadership to facilitate the required decision making processes.
This unit also develops and maintains those required working relationships with all of those relevant organization; agencies; or government departments necessary to advance community interests.
Membership Program
The Membership Program assists in the issuance of Certificate of Indian Status Cards and maintaining an Indian Registry System. Obtaining and examining documentation to verify events related to Indian registration including births, deaths, marriages, divorces and other events and forwarding appropriate documentation to support recommended amendments to the Indian Registrar.
Membership services is provided for the following First Nations:
- Ochiichagwe’Babigo’Ining First Nation
- Wabauskang First Nation
- Wabigoon Lake Ojibway Nation
- All other First Nations must call before accessing membership services

Forms and Requirements
Statement of Parents – Statement of Parents.PDF
This form and a Original Birth Certificate (listing parents names) is needed to apply for Indian Registration.
Guarantor Form – Guarantor’s Declaration.PDF
This form and a certified photo is needed when individuals are applying for a status card and have only (1) one or no identification.
Certificate of Indian Status – Certificate of Indian Status.PDF
This form is needed when applying for a Certificate of Indian Status card. (two pieces of Identification (I.D) is required, one being a photo (I.D).
Lost Declaration – Lost Declaration.PDF
This form is needed when individuals applying for a Certificate of Indian Status card do not have the previsous card at the time of issuance.
Marriage / Surname Change – Marraige – Surname Change.PDF
This form and a copy of marriage, divorce, etc is needed when an individual requests to have their named changed on the Indian Registry System.

This form is required when an individual requests for a band transfer.
Request for Treaty Payment – Treaty Annuity Payment.PDF
This form is available to those who would like request their treaty annuity payment by mail.
